Part 1: Who Bears the Weight
Nine people die from asthma every day in the United States. Almost all of those deaths are preventable.
More than 28 million Americans have asthma, but who develops it, how often they end up in the emergency room, and whose disease is actually controlled are not random outcomes. They follow the same lines — race, income, neighborhood, access to care — so consistently that a map of asthma deaths in America looks like a map of which communities were protected and which were left to breathe whatever air surrounds them.
No group carries a heavier burden than Black Americans, who are roughly twice as likely to die from asthma as white Americans and nearly six times as likely to visit the emergency department. For Black male adults, the mortality gap is even wider — more than three times the death rate of white men — and it has been growing, not shrinking. Black children are nearly four times as likely to die from asthma as the national average for children overall, with emergency department visit rates of 89 per 10,000 compared to 14 for white children. The gap is not narrowing. A 2026 report from the Asthma and Allergy Foundation of America found that the mortality and economic burden has continued to widen despite decades of public health effort.
Puerto Ricans have the highest asthma rates of any population group in the country, far exceeding other Hispanic subgroups — a distinction that aggregate Hispanic statistics routinely obscure. American Indian and Alaska Native adults have among the highest prevalence of any group, compounded by geographic isolation, limited specialty care, and disproportionate exposure to the environmental conditions that trigger attacks.
Adults living below the federal poverty line develop asthma at rates of 12 to 13 percent, compared with 7 to 8 percent at higher incomes — not because poverty causes asthma directly, but because it concentrates people near pollution, in deteriorating housing, and far from the specialist care that keeps the disease controlled. In the Bronx, as many as 17 percent of children have asthma — nearly triple the national average. Detroit, Baltimore, and Bakersfield appear on the same lists year after year.
Women develop asthma at significantly higher rates than men in adulthood, a reversal of the pattern in childhood that standard risk calculators were not designed to explain. For uninsured adults, nearly half report that cost alone prevents them from getting the care their asthma requires. Forty-nine percent. Not inconvenience. Cost.
Approximately 50 percent of children and 62 percent of adults with asthma in the United States do not have their disease well controlled. That is the result of a system that has consistently delivered its best tools to the people who needed them least.

Asthma is a chronic inflammatory disease of the airways, and nearly everything that inflames airways in America is distributed unequally.
The most direct cause of asthma attacks is poor air quality.
The communities bearing the highest asthma burden are disproportionately located near highways, power plants, and industrial facilities — the legacy of discriminatory zoning decisions that concentrated pollution sources in minority and low-income neighborhoods for generations. The air pollution regulations now being rolled back by the current administration were projected to prevent more than 100 million asthma attacks through 2050. That figure is not hypothetical. It comes from the EPA’s own regulatory impact analysis, and the attacks it describes will fall, as they always have, hardest on the communities that were never responsible for producing the pollution.
Inside the home, the triggers are different but equally concentrated.
Mold, cockroach allergens, dust mites, and tobacco smoke are among the most potent drivers of childhood asthma attacks, and they are most common in the older, poorly maintained housing stock where low-income families disproportionately live. The same neighborhood conditions that produce high asthma prevalence also make remediation hardest to access: landlords who resist repairs, code enforcement that is inconsistent, and families without the financial slack to address what they can see is making their children sick.
Climate change makes things worse.
Pollen seasons are longer, wildfire smoke is reaching new geographies, and extreme heat worsens air quality in ways that compound existing disparities. When Canadian wildfire smoke descended on New York State in June 2023, PM2.5 levels exceeded normal by more than 1,200 percent — and emergency department visits for asthma rose 82 percent statewide. The communities that saw the worst of that spike were the ones already living with the worst baseline air quality.
The healthcare system then compounds what the environment started. Without consistent access to primary and specialist care, asthma is managed reactively — in emergency departments, after the attack, when the damage is already done. Approximately 49 percent of uninsured people with asthma report cost as a barrier to care. Controller medications require daily use to prevent attacks, but adherence breaks down when refills are unaffordable, pharmacy access is unreliable, or a complex multi-inhaler regimen demands a level of routine that low-wage, shift-working, housing-insecure families cannot maintain.
Clinical bias adds another layer. For decades, spirometry equations used to measure lung function were adjusted by race in ways that systematically underestimated impairment in Black and Hispanic patients — effectively telling clinicians that patients were less sick than they were, and leaving the highest-burden populations with undertreated disease. Access to allergists and pulmonologists — the specialists whose involvement is most associated with better asthma outcomes — is most limited in the communities with the highest burden, and Medicaid, which low-income patients are most likely to carry, is accepted by fewer specialists than private insurance.
Underlying all of it is the physiology of chronic stress. Sustained exposure to racism, housing instability, neighborhood violence, and economic precarity produces persistent inflammation and altered immune regulation that independently increase both the risk of developing asthma and the severity of attacks. Historically redlined neighborhoods still show higher asthma rates today — not because of anything genetic, but because of what decades of deliberate disinvestment does to bodies over time.

Part 3: What Works — And What’s Being Lost

The tools to close the asthma disparity gap are not hypothetical. They exist, they have been tested, and they work. The question in 2026 is whether they will survive.
The most powerful evidence comes from home visiting programs that deploy community health workers directly into the neighborhoods and households where asthma burden is highest. Boston Children’s Hospital’s Community Asthma Initiative enrolled more than 2,400 children — predominantly Black and Latino children from low-income neighborhoods — and produced an 82 percent reduction in hospitalizations, a 55 percent reduction in emergency department visits, and more than a thousand dollars per year in reduced hospital costs per patient. The program did not invent a new drug or a new procedure. It sent trained workers into homes to identify mold, address pest infestations, improve ventilation, and reinforce medication plans. It moved the healthcare system toward patients instead of waiting for patients to navigate toward it.
Schools are the other critical intervention point for children. The Rochester School-Based Telemedicine Enhanced Asthma Management program — a randomized clinical trial published in October 2025 — added telemedicine specialist consultations to school-based directly observed therapy. The cohort was 91 percent Black and 93 percent publicly insured; more than half had never seen a specialist. Emergency department visits dropped by roughly 50 percent. The program reached children who the conventional healthcare system had never successfully engaged, and it did it by meeting them where they spend most of their time.
In January 2026, the VA and DOD issued updated clinical guidelines formalizing single-inhaler therapy — one device that serves as both daily controller and rescue medication — a change specifically designed to reduce the adherence failures that drive preventable hospitalizations in populations with inconsistent pharmacy access and fragmented care. If Medicaid coverage keeps pace with these guidelines, the change could reach the populations that need it most. That is an open policy question, not a settled one.
These programs have something in common beyond their outcomes. They were built on the recognition that the communities with the highest asthma burden are not failing to manage their disease out of ignorance or indifference. They are managing it under conditions — of poverty, pollution, housing instability, and healthcare scarcity — that make management genuinely difficult. Programs that acknowledge those conditions and work within them produce results. Programs that ignore them produce the numbers we have.
What makes 2026 different is the scale of what is being dismantled at the same time. The CDC’s National Asthma Control Program — which funded state prevention programs, trained school nurses, and supported the infrastructure on which programs like the ones above depend — has been effectively shut down by federal workforce cuts, with funding for state programs approved only through August 31, 2025. Twelve EPA air pollution regulations projected to prevent more than 100 million asthma attacks are under rollback. The 2009 EPA Endangerment Finding — the legal foundation for regulating greenhouse gas emissions — was repealed in February 2026. Medicaid cuts enacted in 2025 are projected to strip coverage from millions of people in a population where asthma is already the most undertreated.
None of this is happening by accident, and none of it will fall equally. The communities already living nearest to the pollution, in the poorest housing, with the least specialist access and the fewest resources to absorb an attack — those are the communities that will absorb the consequences of each rollback. Asthma disparities are not a medical problem awaiting a medical solution. They are a structural problem, and right now the structures are moving in the wrong direction.

If You or Someone Near You Is Having an Asthma Attack
Knowing what to do in the first minutes of an asthma attack can make the difference between a manageable episode and a medical emergency. According to the National Heart, Lung, and Blood Institute, an attack can start with coughing, chest tightness, wheezing, or a feeling of lightheadedness — and you should use your reliever inhaler as soon as symptoms begin.
Sit upright. Lying down makes breathing harder. Stay as calm as you can and breathe slowly.
If you have a serious asthma attack or your symptoms do not go away soon after taking your reliever medicine, seek medical attention. Go to the emergency room if your symptoms do not improve after using your at-home medicines.
You should seek care right away if you or your child have symptoms and have been hospitalized for asthma in the past year, have had a life-threatening attack before, or recently needed oral corticosteroids to treat asthma. Call 911 without delay if breathing becomes so labored that speaking in full sentences is difficult, or if lips or fingernails appear bluish.
Your healthcare provider will probably prescribe a quick-relief inhaler for you to carry at all times — and working with a provider to create a written asthma action plan in advance is the single most reliable way to know exactly what to do when an attack happens.
