Asthma Health Disparities in the United States

An average of nine people die from asthma every day in the United States. Almost all of those deaths are preventable. They are not random.

Asthma is one of America’s most common chronic diseases, affecting more than 28 million children and adults. It can usually be managed well — yet the medications, care, and clean air that make that possible are not shared equally in the United States. The heaviest burden falls on communities with more pollution, worse housing, and less access to care.

Part 1: Who Bears the Greatest Burden of Asthma

Asthma’s burden tracks closely with race, income, and place. That means some groups suffer far more from asthma than the national average.

Black Americans

No group is harder hit. Black Americans experience some of the nation’s highest rates of asthma-related illness and death. Black children and adults are far more likely than white Americans to require emergency care or die from the disease. That gap is not narrowing.

American Indians and Alaska Natives

American Indian and Alaska Native communities experience some of the highest asthma rates in the United States. This disparity affects AI/AN people living either on reservations or in urban areas and is observed across a wide range of income levels. 

 

Puerto Ricans

Puerto Ricans experience exceptionally high asthma rates both on the mainland and in Puerto Rico. Researchers believe a combination of genetic, environmental, social, and healthcare factors contributes to this persistent disparity.

Low-income families 

Asthma is more common among people with lower incomes. Children and adults living in poverty are more likely to experience severe symptoms, emergency room visits, hospitalizations, and disruptions to school, work, and daily life. 

Part 2: Why Do These Asthma Disparities Exist?

Asthma is a chronic inflammatory disease of the airways. The conditions that fuel it are experienced unequally in the United States. 

1. Outdoor Air Pollution and Environmental Injustice

Low-income communities and communities of color face higher exposure to fine particle pollution, diesel exhaust, ozone, and other pollutants that increase asthma risk and trigger attacks. They are disproportionately located near highways, freight corridors, ports, industrial facilities, and other sources of dirty air, a pattern shaped by decades of discriminatory housing, zoning, and transportation policies.

What can be done: Strengthen emissions controls near homes and schools, reduce traffic-related pollution, and require zoning and permitting decisions to consider the cumulative pollution burden already borne by nearby communities.

In a landmark 2022 study, Harvard researcher Francesca Dominici and her colleagues mapped historic fine-particle air pollution (PM2.5) levels across 32,000 U.S. ZIP codes from 2000 to 2016.

2. Housing and Neighborhood Conditions

Substandard housing exposes many families to mold, cockroaches, rodents, dust mites, and secondhand smoke that increase asthma risk and trigger attacks. These hazards are more common in older, poorly maintained housing, where low-income renters often have limited power to secure repairs or move to healthier environments.

What can be done: Expand healthy-housing repair programs, strengthen housing code enforcement, and ensure landlords address asthma-triggering conditions such as mold, pests, and water damage.

From Mold to Asthma to Homelessness

Angel and Dedric Harris’s story is a too familiar one: how substandard housing can turn asthma into a life-upending crisis. Within months, the Milwaukee home they rented in late 2020 was leaking water through ceilings and walls, with black mold creeping room to room. Both adults had asthma, and turning on the heat only spread the spores.

As Angel’s symptoms escalated, she relied on two inhalers, a nebulizer, and repeated hospital visits, and the couple sent all five asthmatic children to live with relatives. Their complaints went nowhere. Milwaukee doesn’t regulate mold, so the landlord was never required to remove it. After withholding rent in protest, the Harrises were evicted in 2022 and left homeless.

Elizabeth Matsui of the Johns Hopkins Bloomberg School of Public Health and her colleagues demonstrated that allergens found in mouse urine are widespread in inner-city homes and schools and are strongly associated with asthma symptoms, attacks, emergency visits, and reduced lung function among sensitized children.

Her work  highlighted how environmental conditions common in underserved neighborhoods can worsen asthma and deepen health inequities.

Poverty and chronic stress

Poverty influences nearly every major driver of asthma disparities, from where families live and what they breathe to whether they can obtain consistent medical care. Chronic stress from financial hardship, violence, housing insecurity, and discrimination can trigger inflammatory responses that make airways more reactive, increasing the risk of symptoms, severe attacks, and poor asthma control.

What can be done: Expand income and housing supports, reduce neighborhood stressors, and integrate stress and trauma screening into pediatric and primary care.

How Climate Change Fuels Asthma

Lewis Ziska, an environmental health researcher at Columbia University’s Mailman School of Public Health, has done more than perhaps any single researcher to establish how climate change worsens allergies and asthma.

His greenhouse experiments showed that a doubling of atmospheric carbon dioxide can boost ragweed pollen production by 60 to 100 percent per plant, proving that CO₂ acts as a fertilizer for allergenic weeds. His field studies demonstrated that global warming has lengthened ragweed seasons and made pollen itself more allergenic.

Ziska’s work laid the foundation for understanding how a warming climate falls hardest on those already struggling to breathe. Read more

4. Healthcare Barriers

Many families struggle to obtain the care needed to keep asthma under control. Some lack a consistent primary care provider, while low Medicaid reimbursement can make it difficult to find specialists who accept their insurance. Medication costs and prior authorization requirements can further delay treatment. As a result, asthma is often treated after attacks occur rather than prevented through regular care. For many families, the emergency room becomes the primary source of asthma care.

What can be done: Expand access to primary and specialty care, raise Medicaid reimbursement, reduce unnecessary prior authorization requirements, and lower out-of-pocket medication costs.

This is the actual title of a 2023 study on how families absorb the cost of asthma care. When asthma medication is unaffordable, families don’t simply go without. They make hard substitutions, borrowing from one need to cover another.

  • They borrow money or cut back on necessities.
  • They take on debt.
  • Parents go without their own care so the child gets medication.
  • Children use less medication than prescribed.
  • They substitute alternative therapies for prescribed medicine.
  • They shop the system to shave the price.

     

5. Substandard Clinical Care Delivery and Systemic Bias.

Medical access does not guarantee equitable treatment. Even when insurance and income are held constant, Black and Latino patients face lower-quality clinical care—delayed diagnosis, fewer specialist referrals, under-prescription of controller medications, and underuse of standard asthma action plans. These are institutional delivery failures, compounded by implicit biases that lead clinicians to underestimate symptom severity in patients of color.

What can be done: Mandate guideline-based asthma protocols, track quality metrics stratified by race, and embed anti-bias training and community health workers as structural extensions of clinical care. 

How Climate Change Fuels Asthma

Since 2015, the American Lung Association’s Asthma Care Coverage Initiative has audited all 50 state Medicaid programs, mapping gaps in coverage of guidelines-based care.

Using that data, the ALA lobbies state Medicaid agencies and Congress to remove barriers like prior authorization and step therapy and to protect funding—work that disproportionately benefits the low-income children of color who rely on Medicaid.

Part 3: Asthma Solutions That Work

Disparities did not arise overnight and will not close through better drugs alone. But some  proven approaches can move the numbers. Here are four examples of what works. More are found in our Asthma Solutions Library. 

Clean-air policy. New York City’s Clean Heat Program required thousands of buildings to drop the dirtiest heating oils for cleaner fuels.
Impact: This prevented an estimated roughly 780 deaths and 1,600 asthma ER visits a year, with the biggest gains in the dirtiest-air neighborhoods.

Healthy-housing remediation. Baltimore’s Green & Healthy Homes Initiative replaced one-off repairs with comprehensive fixes such as structural repair, mold remediation, pest control, weatherization.
Impact: A  66% drop in asthma-related hospitalizations, returning an estimated $5–$14 for every dollar invested.

Community health workers. Boston Children’s Community Asthma Initiative sent community health workers into homes for education, home assessment, and trigger removal.
Impact: Hospitalizations fell about 85% and ER visits about 68%, returning $2.56 for every $1 spent.

Cost caps and coverage. After a Congressional investigation, three major manufacturers capped out-of-pocket inhaler costs at $35 a month for the commercially insured and uninsured; some states have gone further with cross-brand caps in regulated plans.
Impact: TK, although the gap that remains for many Medicaid and Medicare patients.

A Final Word

 

Asthma is one of the most manageable chronic diseases in medicine. The drugs work. The interventions work. The evidence on what drives disparities—and what reduces them—is clear. The challenge is no longer understanding the problem. It is applying the solutions.

Act immediately. Use a quick-relief (rescue) inhaler as soon as symptoms begin. Sit upright, as lying down can make breathing more difficult, and try to stay calm while breathing slowly and steadily. If symptoms do not improve soon after using the inhaler, seek medical care.

Call 911 immediately if breathing becomes so difficult that the person cannot speak in full sentences, if the lips or fingernails appear bluish, or if symptoms are rapidly worsening.
