The New York-Presbyterian and Weill Cornell Medicine’s Community Tele-Paramedicine (CTP) program aims to address health disparities by providing comprehensive, personalized care to underserved patients with heart failure. The program combines telemedicine with in-person care, allowing specially-trained paramedics to conduct home visits and perform on-site evaluations and treatments. These paramedics are supervised in real-time by board-certified emergency medicine doctors via video chat, ensuring high-quality care.
The CTP program can perform EKGs, blood tests, administer medications, and prescribe new medications electronically. By bringing emergency department-level care to patients’ homes, the program reduces the need for hospital visits, which can be a significant barrier for underserved populations. Additionally, the program helps coordinate care, set up appointments, and arrange additional services or transport, further supporting patients in managing their health effectively at home. This approach not only improves access to care but also enhances the quality of care for patients who might otherwise face significant challenges in receiving timely and appropriate medical attention.
